1. A diagnosis of bipolar disorder or schizophrenia with any history of violence.

2. High-risk substance use, including:

– Benzodiazepine use greater than ~6 mg Xanax-equivalent from non-prescription sources

– Ongoing, uncontrolled alcohol or drug use

– Fewer than 6 months of sobriety

3. Significant cognitive impairment without a family member or support person willing to be actively involved in my care
